Free and Reduced Price Lunch: We will be contacting you later in the summer when the new federal forms are available.

Please submit all forms within two weeks of receiving notification of your acceptance. If you were accepted before July 1st, 2009, please submit all forms by July 15th, 2009 or you risk being moved to the bottom of our waiting list. See our Waiting List Policy below for more details.

  • Permission for Public Use: Please tell us whether or not you and your parent/guardian consent to having your image or work publicly displayed. Please note that only your first name will be given on any internet-accessible media.
  • Transportation Request: If you live within 15 miles of New Roots Charter School, your home school district is legally required to provide your transportation. To request this, regardless of what district you live in, you may use the Ithaca City School District form located below. Simply cross out the district name and write in your own district. Submit forms to us for the Principal's signature, and we will turn them in to the district for you.
  • Records Release: Please fill out, have your parent/guardian sign, and return to us so we may request your records from your current school.
  • Residency Verification: Print the "Residency Verification Form" and send it to us along with the documentation it mentions to establish your residency. You do not need to send us the "Residency Verification Policy-Practice" document; that is just for your information.
  • Health History: Print this, fill it out, and return it with your immunization records. You can find out what immunizations are required at

NEW ROOTS CHARTER SCHOOL
WAITING LIST POLICY


I. POLICY
a. ENROLLMENT DURING THE SUMMER PRIOR TO OPENING (2009)
Our policy in the summer of our opening year is to enroll students on a first come, first served basis, as there was no need for a lottery on April 1st, 2009. Any student who is eligible to attend public school in New York State can enroll at New Roots Charter School if her or his enrollment form is submitted before the first day of classes (September 9th, 2009) and there is still space available.

In future years, if there is a need for a lottery, our policy will afford the preferences set forth in the Charter Schools Act for siblings, returning students, and students residing in the district of location (ICSD). Otherwise, there are no admission criteria and no selection process.

b. ENROLLMENT ONCE CLASSES HAVE BEGUN
Once classes have begun, it is our policy that new students may only enroll at the start of a term. Exceptions to this requirement may be considered by the Principal if exceptional circumstances dictate.


II. PRACTICE
Because we are approaching a waiting list situation, as of Wednesday, July 1st the enrollment practice will be as follows.

a. ENROLLMENT AND WAITING LIST
Students wishing to enroll at New Roots Charter School will submit an enrollment form and will either be accepted (if space is still available) by the school or put on a waiting list, and informed of their status. The waiting list will be maintained in the order the enrollment forms were received.

Students who are accepted must confirm their intent to enroll by submitting all paperwork requested by the school. Students who have not submitted a Records Release Form and completed their Residency Verification within two weeks of receiving notice of acceptance, or before the first day of classes (whichever is first) may be moved to the bottom of the waiting list, and their seats be offered to students currently at the top of the list who have confirmed their desire to enroll by submitting all required forms. (At the principal's discretion, exceptions may be made for extenuating circumstances where the student has made his or her intent to enroll clear in other ways).

Students on the waiting list are also encouraged to submit a Records Release and Residency Verification as a way of pre-confirming their intent to enroll. (Records Release Forms will not actually be sent to the student's current school until he or she has been notified of acceptance at New Roots). Students who are accepted off the waiting list after July 1st have two weeks from being notified of their acceptance to complete the same paperwork. Students who were accepted prior to July 1st have two weeks from July 1st to submit a Records Release and complete their Residency Verification.

To enroll during the school year, the Enrollment Form, Records Release, and Residency Verification must be submitted before the first day of the term in which the student is planning to attend for the first time.

b. WITHDRAWAL
Once the Records Release has been submitted and the Residency Verification has been completed, a student wishing to withdraw must do so in writing, either via a paper letter or a fax. A parent or guardian's signature (or the student's signature if the student is not a minor) on this document is needed to confirm that we may give away the student's seat at the school. If there are extenuating circumstances that prevent a parent or guardian from withdrawing in writing, the Dean of Students may consider a student as having withdrawn if there is other documented evidence that the student does not intend to enroll, upon approval by the Principal.
